Access Your Clinical Patient Portal

To access your Clinical Patient Portal, please click the “Clinical Patient Portal Login” and follow these directions

1. Click the “Practice Code/Phone” radio button as seen below

2. Then enter CBIIAD as the Practice Code and click GO
